I understand that if I have not submitted payment in the form of cash or check by 3:00 pm to the Heritage Center office, the following credit/debit card will be run for the payment due. A late fee of $15.00 will be assessed if not paid on time or card is declined.

I certify that I am the legal cardholder of the account listed above, and I authorize HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH to initiate debit and credit entries, as well as any necessary correction entries to the account listed above to pay my fees. Should I dispute a charge through my credit provider this will constitute a breach of this agreement. If a Credit Card payment to HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH is refused or revoked for any reason, I acknowledge and agree that I am nonetheless personally liable to HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH for the amount of such payment in accordance with the written agreement(s) between me and HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH. I agree that if any such withdrawal is dishonored with cause, HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH shall be under no liability whatsoever, if such dishonor results in late charges or revocation of my card. This authority is to remain in full force and is effective until HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH has received written notification from me of any payment or termination in such time and in such manner as to afford HERITAGE CENTER FOR RELIGIOUS STUDIES OR THIRD NEW HOPE BAPTIST CHURCH a reasonable opportunity to act on it.
